  1. The promoter is: the Ontario Physiotherapy Association whose registered office is at 55 Eglinton Ave E, Toronto, ON M4P 1G8.
  2. Employees of the Ontario Physiotherapy Association and Bond Association Management shall not be permitted to enter the contest. Members of the Ontario Physiotherapy Association are permitted to take part in the contest.
  3. The five individuals on a team must be registered for the full conference InterACTION 2019 and the contest form must be completed by Monday, April 1, 2019 at 11:59pm.

  5. Closing date for entry will be Monday, April 1, 2019 at 11:59pm. After this date no further entries to the contest will be permitted.
  6. No responsibility can be accepted for entries not received for whatever reason.

The promoter reserves the right to cancel or amend the contest and these terms and conditions without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside of the promoter’s control. Any changes to the contest will be notified to entrants as soon as possible by the promoter.
  9. The promoter is not responsible for inaccurate prize details supplied to any entrant by any third party connected with this contest.
  10. Prizes are subject to availability and we reserve the right to substitute any prize with another of equivalent value without giving notice.
  11. Winners will be chosen through a random prize draw.
  12. The winning team members will be notified on April 6, 2019 at the Awards Dinner and Gala. Any winning team members that are not able to attend the Gala will be notified via email on April 9, 2019. One winning team member must be present at the Awards Dinner and Gala in order to collect the prize. If there is not a winning team member present at the Awards Dinner and Gala, we reserve the right to withdraw the prize from the winner and pick a replacement winner.
  13. The promoter will notify the winner when and where the prize can be collected.
  14. The promoter’s decision in respect of all matters to do with the contest will be final and no correspondence will be entered into.
  15. By entering this contest, an entrant is indicating his/her agreement to be bound by these terms and conditions.
  16. The contest and these terms and conditions will be governed by Canadian law and any disputes will be subject to the exclusive jurisdiction of the courts of Canada.
  17. The winner agrees to the use of his/her name and image in any publicity material. The winners agree to the use of their names and photo in any and all OPA communications and / or promotional materials. Any personal data relating to the winner or any other entrants will be used solely in accordance with current Canadian data protection legislation and will not be disclosed to a third party without the entrant’s prior consent.
  18. Entry into the contest will be deemed as acceptance of these terms and conditions.
  19. By entering this contest, you are consenting to receiving OPA communications, from which you can unsubscribe at any time.
