I hereby grant to Ontario Physiotherapy Association, its successors, assigns and licensees the perpetual right to use, as you may desire, all writing, video and photography submitted, and the right to use my name or likeness in or in connection with the exhibition, advertising, exploiting and/or publicizing of the picture and writing. I agree that I will not assert or maintain against Ontario Physiotherapy Association, your successors, assigns and licensees, any claim, action, suit or demand of any kind or nature whatsoever, including but not limited to those grounded upon invasion of privacy, rights of publicity or other civil rights, or for any reason in connection with your authorized use of my writing or photography submitted to be published via email, website or social media profiles of the Ontario Physiotherapy Association. I have received permission from any other individuals other than myself included in the picture(s) to share their image and likeness. I have received permission to include any logos or other identifiers of the organization in the picture(s). I hereby certify and represent that I am over 18 years of age and have read the foregoing and fully understand the meaning and effect thereof.
