We are seeking a Physiotherapist (PT) to join a multi-disciplinary team to provide care in a primary care clinic. The clinic is in the two communities of Wasaga Beach and Collingwood in modern, purpose-built settings. You will facilitate the continuity of primary health care and health promotion by working collaboratively with our nurse practitioners, local physicians, and Interdisciplinary Health Professionals including, but not limited to, the Georgian Bay Family Health Team, South Georgian Bay Community Health Centre, Georgian Bay Family Health Organization.

Key responsibilities:

  • Use research and evidence to inform clinical care, education, and planning.
  • Collaborate with members of the community and an interdisciplinary team to plan, implement, coordinate, and evaluate patient care, in consultation with individuals and their families.
  • Advise patients and their families on health care including health promotion, disease, illness, and accident prevention.

Qualifications and Education:

  • Bachelor’s degree in health sciences, holding current registration with the College of Physiotherapists of Ontario in good standing.
  • Minimum 5 years’ experience
  • Experience with community care and/or a primary care setting.
  • Hold current First Aid and CPR certifications.

Requirements, skills, and knowledge:

  • Valid Ontario driver’s license and operational vehicle.
  • Models and promotes core ethical practice and reflects an optimistic and positive attitude.
  • Experience in caring for older adults preferable.
  • Be adept at working collaboratively with members of our interprofessional team, family physicians, nurse practitioners, and other community health professionals.
  • Have a solid knowledgebase of local community supports for families and caregivers.
  • Demonstrated competency required in:
  • Interpersonal engagement and team building.
  • Formulating and implementing individual treatment plans based on analysis of assessment findings and clinical reasoning.
  • Decision-making, critical thinking and conflict resolution.
  • Risk management and optimizing patient safety.
  • Managing competing demands and priorities within defined timelines.
  • Verbal and written communication, including effective use of computer programs like Microsoft Office, PS Suite experience an asset.
  • Working autonomously and being self-directed.
